Loose fitting, comfortable clothing is best, so that your therapist can easily examine the area to be treated. For example, if you are receiving treatment for your knee, it is best to wear shorts or pants that can be easily rolled up.
What Do I Need to Bring With Me?
Please bring your physical therapy referral provided to you by your doctor.
Insurance card(s), if your insurance is covering the cost of physical therapy.
Identification card, driver’s license or other.
Completed registration forms. Registration forms are available on our Patient Center page of this website or can be emailed to you upon request. You may also fill the forms out at our office at the time of your visit, but we ask that you arrive 15 to 20 minutes prior to your appointment to do so.
How Long Will Each Sessions last?
Your initial evaluation will typically last approximately 60 minutes.
Follow up treatment sessions are typically between 30 and 45 minutes.
How Many Visits Will I need?
The number of visits you will need will depend on your diagnosis, the severity of your impairment, and any past medical history. You will be re-evaluated on a monthly basis by your physical therapist who will communicate with your doctor to provide recommendations on the number of visits you will need.

Can I Use My Medical Insurance?
How Much Will Physical Therapy Cost?
The cost of therapy will vary depending on which insurance you have. As a courtesy to our patients, we do contact your insurance company prior to your first visit to find out if you will have a deductible, co-pay or co-insurance. However, we highly encourage our patients to contact their insurance company as well to find out the specifics of their particular insurance plan.
Which Insurances Do You Accept?
We accept most PPOs, Medicare, and Worker’s Compensation Insurances. Please contact our clinic for a current list of insurances we accept.
Unfortunately, at this time we do not accept HMOs or Medi-Cal. We do offer competitive cash rates if you would like to receive services with us.
Do I Need a Prescription For Physical Therapy?
We encourage all of our patients to see their physician for a physical therapy prescription prior to coming in for therapy. California allows for Direct Access to physical therapy without seeing an MD first, however in many cases insurance companies will still require a prescription from an MD.
Will Insurance Cover Wellness Services?
At this time, most insurance companies do not cover the wellness services we offer (Laser Therapy, Fitness, Athletic Training, Massage, Yoga, or Pilates). We do, however, offer competitive cash rates and package deals for these types of sessions. For a current list of prices, please contact our office.
Good Faith Estimate
Evergreen Physical Therapy Specialists, Inc. will provide all patients who do not have insurance or who are not using insurance an estimate of the bill for therapy services. Please retain a copy of your “Good Faith Estimate” (GFE) quote.
Our initial estimate will be for the cost of the first visit, which will be for the evaluation. This estimate will be provided to you at least one (1) business day prior to the evaluation or prior to scheduling, if you prefer.
Before commencing with a treatment, a second estimate will be provided to you based on the established Plan of Care. If your Plan of Care needs modification during the episode of care because of your therapy needs, a new condition, or per your request, you will be informed and provided with a new estimate, and you will have the option to continue or cease treatments.
If you receive a bill that is $400 or more than the “Good Faith Estimate,” you can dispute the bill utilizing the Patient-Provider Dispute Resolution Process. The dispute resolution entities (SDR) are certified and selected by Health & Human Services (HHS). You must submit a dispute notice, the provider’s bill, and the “Good Faith Estimate” to HHS within 120 days of receiving the final bill from our practice. HHS will collect a $25 fee with your dispute resolution application.
