Evergreen Physical Therapy Specialists is partnering up with FOTO, Inc. to better serve you and your physical therapy needs. FOTO, Inc. is the largest and most valid external database for rehabilitation. FOTO measures functional outcomes for musculoskeletal, neuromuscular and cardiopulmonary patients by administering quick 10 minute online surveys. The surveys ask questions about pain, functional ability and are specifically designed for each patient based on your diagnosis. FOTO, Inc outcome surveys assist your physical therapist in better serving you and your physical therapy needs.
